What information do you need to make plans for your discharge or the discharge of your family?
Discharge planning is a process which begins with the patient's admission and works cooperatively with the patient, physician and clinical staff to plan for a safe discharge to a location which permits the least confined level of care and the safest outcome.
Sutter Delta's Case Managers are available to assist you with your planning needs following a hospital episode. Case Managers are knowledgeable nurses and health care providers who are aware of the community resources and services, which will help you continue your recovery and maintain your health. Sutter Delta has several case managers to help you. Each is assigned by location of the patient. They are:

Skilled Nursing Facilities
Sometimes erroneously referred to as nursing homes, these facilities provide skilled nursing care and rehabilitation.
In Eastern Contra Costa County, there are four (4) licensed Skilled Nursing Facilities:

Insurance coverage is historically limited to those patients needing skilled nursing care as documented by standards of Medicare (Health Care Financing Administration). Traditionally, many persons are not aware that Medicare coverage for skilled care is not unlimited and often is for a short period. Each of the facilities that we have listed as Skilled nursing facilities, which our medical staff utilizes, will be happy to explain the limitations and the expected or anticipated costs involved in placement at their facility.If you are insured with an HMO form of insurance these choices may be limited to the facilities, which have contracts for care.
